Died of wounds, at 1 Military Hospital, received while on patrol with his men in a SWA-built Wolf mine-protected vehicle, they drove into an ambush. Sat with his head against the side of the passenger compartment, Charlie was fatally wounded when an Energa anti-tank rifle grenade struck the vehicle right where his head was. Of those evacuated to 1 Military Hospital, he was one of only a few who succumbed to his wounds. (Source General Georg Meiring)
